Requirements

• One (1) year of RN experience in the local market.
• Strong knowledge of governmental home health regulations and Medicare requirements.
• Experience in care plan development techniques.
• Currently licensed as a registered nurse in the state of Texas.
• Must possess a valid driver's license and have access to an insured automobile.

Responsibilities

• Serve as the primary point of contact for patients and families in the hospice care process.
• Develop and implement care plans in accordance with Medicare requirements and governmental regulations.
• Collaborate with healthcare professionals to ensure seamless patient care transitions.
• Educate patients and families about hospice services and support available to them.
• Maintain accurate documentation and records of patient interactions and care plans.
