Responsibilities

  • Act as an inpatient rehabilitation expert between internal and external referral sources and the inpatient rehabilitation program.
  • Ensure appropriate patient admissions to the inpatient rehab unit through clinical assessments and screenings in collaboration with the rehab physicians and Medical Director.
  • Focus on moving appropriate candidates into the inpatient rehabilitation unit at the appropriate time.
  • Work closely with hospital case management to ensure a responsive and smooth flowing referral and admission process for the inpatient rehab program.

Requirements

  • Current license to practice as RN or RN case management experience.
  • 1 year of acute rehab experience preferred.
