About The Position

Provide strategic clinical leadership and operational oversight for primary care provider services, with a strong emphasis on clinical program development and the integration of value-based care principles into daily provider practice. This role is responsible for advancing exceptional patient outcomes, supporting sustainable and profitable growth, ensuring regulatory and compliance integrity, and driving continuous clinical quality improvement across 360Care’s primary care service lines.

Requirements

  • Current license for specialty with 10+ years of experience, preferably in primary care.
  • 3+ years of Primary Care leadership experience.
  • Must pass credentialing, maintain reliable transportation, insurance, malpractice coverage, and pass background/drug screening.
  • Strong documentation and communication skills.
  • Excellent interpersonal, oral, and written communication skills.
  • Must be detail oriented and self-motivated.
  • Excellent customer service skills.
  • Anticipate needs in a proactive manner to increase satisfaction.

Responsibilities

  • Establish and oversee the clinical vision for primary care-focused general practitioner services.
  • Lead the design, implementation, and ongoing optimization of primary care clinical programs, care pathways, and standardized workflows.
  • Partner with executive leadership to align clinical strategy and value-based care initiatives with organizational and financial objectives.
  • Identify, operationalize, and continuously refine best practices that advance clinical quality, care coordination, and population health outcomes.
  • Collaborate with executive leadership to identify, implement, and embed meaningful clinical outcome measures into primary care workflows to support quality improvement and value-based care initiatives.
  • Develop and oversee clinical quality standards and protocols aligned with value-based care models and primary care performance metrics.
  • Ensure all services comply with regulatory requirements, industry standards, and payer expectations for quality and outcomes.
  • Coordinate with legal and compliance teams to stay current on evolving care delivery, quality reporting, and reimbursement regulations.
  • Support recruitment, onboarding, and ongoing professional development of providers, with an emphasis on primary care competencies and value-based care workflows.
  • Provide education and guidance to clinical teams on the practical application of value-based care principles in daily practice, including risk-adjusted documentation and HCC coding, care coordination, preventive care, and accurate documentation of chronic conditions to support population health management.
  • Lead and oversee clinical education initiatives, including workshops, clinical program rollouts, and targeted provider coaching to support consistent, high-quality practice.
  • Ensure that high-quality, patient-centered primary care is consistently delivered across all clinical settings.
  • Resolve patient feedback and clinical concerns with a focus on improving care continuity, engagement, and outcomes.
  • Partner with customer service teams to reinforce exceptional customer and patient experience within primary care programs.
  • Partner with finance and business operations to develop and manage clinical budgets aligned with primary care and value-based performance goals.
  • Collaborate with business development leadership to design, expand, and scale primary care clinical programs that support sustainable growth and evolving care delivery models.
  • Analyze primary care performance data to identify opportunities for improvement in quality, clinical outcomes, operational efficiency, and cost-effectiveness.
  • Work collaboratively with billing and revenue cycle teams to identify coding trends and denials, using insights to inform provider feedback, education, and documentation improvement initiatives.
  • Develop and guide a network of Regional Clinical Directors to support implementation of primary care programs and value-based care workflows.
  • Provide leadership to ensure seamless integration of primary care initiatives into operations, clinical processes, and provider routines.
  • Work collaboratively with the HER and clinical informatics teams to refine documentation workflows and tools, improving end-user experience, data accuracy, and clinical efficiency.
  • Promote cross-functional collaboration with operations, sales, and support teams.
  • Stay current on advancements in primary care delivery, value-based care models, and mobile care innovations.
  • Support adoption of new technologies that enhance care coordination, clinical efficiency, and patient engagement.
  • Drive innovation in care models, particularly those that support preventative care, chronic disease management, and population health.
  • Identify clinical risks associated with primary care operations and program implementation.
  • Develop and monitor risk-mitigation strategies tied to quality measures, patient safety, and value-based performance indicators.
  • Partner with the risk management team to align strategies across the organization.
  • Report directly to the Chief Clinical Officer and serve as clinical leader for Regional clinical Branch Directors.
  • Provide ongoing updates to the executive team regarding primary care program performance, value-based metrics, and overall clinical outcomes.
  • Represent the company in external forums, conferences, and panels focused on primary care transformation and value-based care.
  • Support all company policies, procedures, compliance guidelines, and the Employee Handbook.
  • Monitor regulatory and payer requirements specific to primary care and educate providers accordingly.
  • Maintain accurate clinical records using electronic medical systems.
  • Support positive relationships with patients, facilities, and coworkers.
  • Demonstrate effective communication, multitasking, and customer service skills.
  • Actively supports and complies with all components of the compliance program, including, but not limited to, completion of training and reporting of suspected violations of law and Company policy.
  • Maintains confidentiality of all information; abides with HIPAA and PHI guidelines at all times.
  • To work remotely, it is expected you will have a reliable, nonpublic high-speed internet connection with sufficient bandwidth to participate in all work-related activities.
  • Any interruptions in service should be immediately reported to your manager.
