About The Position

The Clinical Intake Specialist is an integral member of the Enhanced Care Management (ECM) team. In this role, the Clinical Intake Specialist works directly with clients to understand their needs and then connect them to the correct resource on our primary care management team. The successful candidate will be personable and professional via phone, able to quickly review medical charts and understand medical conditions, provide person-centered care, prioritize tasks effectively to support client engagement, and be motivated to help high need individuals with complex care needs.

Requirements

  • Active, unrestricted California Licensed Vocational Nurse (LVN) license, or Bachelor’s degree in a related field, or Medical Assistant certificate issued by a recognized group (AAH, AAMA, AMT, NCCT, or NHA)
  • Minimum of 2 years of previous intake or admissions experience in a high call volume environment
  • Experience working with complex care clients and ECM Populations of Focus
  • Experience with Health Gorilla or Health Record Systems
  • Strong oral and written communication skills
  • Positive interpersonal skills with a collaborative attitude
  • Expert computer skills and proficiency in Google Workspace, MS Office, and internet tools
  • Ability to work independently and stay highly motivated
  • Capable of managing multiple tasks and navigating various platforms, with the ability to adapt and shift priorities as needed
  • Willing and able to work Monday-Friday 8:30am-5pm Pacific Time, with the possibility of overtime as needed.

Responsibilities

  • Conduct outreach to newly referred clients and log activity in Salesforce
  • Conduct 8-12 intake visits daily, ensuring efficient and accurate assessments
  • Assist clients in completing initial intake visit including a psychosocial assessment and care plan with goals of care
  • Review medical charts and document client medical needs and suggested interventions
  • Communicate critical information and client preferences to the care team for personalized care planning
  • Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
  • Attend weekly Team Operations Meetings
  • Maintain a structured and organized schedule to optimize client intake and engagement
  • When unscheduled, perform proactive outreach to no-shows or clients who have not been contacted previously
  • Stay available for warm transfers from field teams, ensuring same-day responses
  • Thrive in a fast-paced, dynamic environment with an emphasis on meeting key performance metrics
