The Referral Liason/Intake Specialist will establish and maintain collaborative relationships with area referral sources, including hospitals, schools, courts/correctional facilities, and others as indicated by necessity.The Referral Liason/Intake Specialist will maintain data set to ensure agency adherence to OMH hospital discharge/follow-up appointment regulations and inform internal strategic decision-making. The Referral Liason/Intake Specialist will exercise clinical judgement in advocacy and inquiry to promote adherence to best practices in hospital discharge/care transition planning, and will apply this skillset to initiating internal care planning and care coordination.The Referral Liason/Intake Specialist will balance this administrative/coordination workload with their participation in the direct service aspect of the intake process, including the completion of Initial Assessments and Initial Treatment Plans. As outlined above, this role blends direct clinical service with administrative and communications functions. The Referral Liason/Intake Specialist must have an established clinical skillset enabling the completion of intake clinical functions, as well informing productive case consultation and parsing of referral documentation. The administrative and communications functions require strong interpersonal skills, strong verbal and written communication, and strong organizational skills and attention to detail. In functioning as Referral Liason, this clinician will maintain and develop productive relationships with key players among frequent referral sources, promoting mutually beneficial outcomes for Access Behavioral Health programs, our clients, and our community partners along the care/service continuum. The Referral Liason/Intake Specialist will serve a critical role as a primary outward-facing point of contact, representing our expansive Access Behavioral Health programs in interacting with community partners along the care/service continuum. As such, role will receive organizational support from Behavioral Health leadership, and will work collaboratively with program and site leadership from among our various programs.
