  • Evaluates medication therapy for appropriateness, taking into account relevant patient conditions including age, allergies, renal status, and other disease states.
  • Provides therapeutic drug level monitoring when appropriate.
  • Effectively communicates with nurse/prescriber when errors are detected or to optimize therapy.
  • Consults with other practitioners as appropriate.
  • Identifies trends during routine provision of pharmaceutical care, collects data supporting concern, and formulates practice recommendations.
  • Presents data to the appropriate committee as required.
  • Documents all appropriate clinical consult information in the medical record using standard hospital guidelines.
  • Demonstrates accuracy when dispensing or preparing medications.
  • Identifies ways to decrease the possibility of medication dispensing errors.
  • Completes tasks with appropriate speed yet not sacrificing accuracy.
  • Demonstrates reliability and performs tasks in a conscientious manner.
  • Demonstrates effective time-management skills and meets deadlines.
  • Communicates inability to complete tasks to supervisors in a timely manner.
  • Performs routine duties without direct supervision and brings concerns to appropriate superior.
  • Develops the skills of others and may be responsible for orientation of new personnel.
  • Makes presentations at local or national meetings or publishes in internal/external publications.
  • Exhibits proficiency in the use of area-specific technology and techniques.
  • Complies with required documentation activities such as Adverse Drug Reactions and Clinical Pharmacy Interventions.
  • Supervises supportive personnel assuring efficient operation of the area.
  • Generally requires 5 to 7 years of related experience.
  • Pharm.D. degree and residency training or equivalent proven experience is required.
  • Possession of or eligible for Florida pharmacist license is required.
  • Prior experience in Oncology/Hematology setting.
