About The Position

The Lead Home Infusion Pharmacist is responsible to supervise, lead, evaluate, and develop the clinical operations of the IV department while ensuring the highest level of patient care from a clinical perspective. This role emphasizes clinical excellence, focusing on patient safety, treatment efficacy, and regulatory compliance. Reporting directly to the Production Director and under the clinical and regulatory oversight of the Pharmacist in Charge, this position requires effective management of Clinical Home Infusion Pharmacists and seamless coordination of patient care. This position is a critical leadership role, ensuring the IV department not only meets but exceeds standards of patient care, safety, and compliance. The Lead Home Infusion Pharmacist is a pivotal figure in fostering a culture of excellence, collaboration, and continuous improvement within the pharmacy team.

Requirements

  • Ability to comprehend alternatives and make efficient decisions.
  • Must possess current knowledge of all applicable state and federal pharmacy laws, rules and regulations.
  • Microsoft Office proficiency.
  • Excellent Customer Service and communication skills.
  • General mathematics skills.
  • Drug nomenclature and/or medical terminology knowledge.
  • Infusion supply knowledge.
  • Able to simultaneously manage and prioritize multiple complex projects.
  • Solid attention to detail.

Responsibilities

  • Manage and oversee the clinical functions of the IV department to ensure high-quality patient care.
  • Provide direct support to Home Infusion Pharmacists, stepping in as needed to guarantee optimal patient outcomes.
  • Efficiently manage both ongoing and newly onboarded patients.
  • Review medication orders for accuracy, clinical appropriateness, and safety.
  • Conduct and review patient assessments and Care Plans.
  • Resolve patient concerns and complaints promptly and effectively.
  • Provide patient counseling when necessary.
  • Communicate with healthcare providers regarding patient status and treatment adjustments.
  • Assume on-call responsibilities when required.
  • Perform other duties as assigned by the supervisor.
  • Manage daily clinical workflow to meet department deadlines.
  • Train, evaluate, and maintain Pharmacist competencies.
  • Manage department scheduling and approve time-off requests.
  • Conduct periodic patient profile audits to ensure clinical appropriateness of medication orders.
  • Ensure compliance with payor regulations.
  • Document medication errors for Quality Assurance review.
  • Report product incidents and unresolved patient complaints.
  • Develop clinical department guidelines and protocols as required.
  • Participate in performance improvement programs and inspection agency surveys.
  • Pursue continuing education relevant to job responsibilities and licensing requirements.
  • Ensure patients are residing in a pharmacy licensed state.
  • Report unresolved patient complaints, medication errors, delivery delays, and regulatory noncompliance to the Pharmacist in Charge.
  • Report interdepartmental issues, staff errors, task delays, and performance concerns to the Director of Production.
  • Represent the company professionally in all job-related interactions.
