About The Position

The Clinical Quality Review Specialist is a remote RN position responsible for processing and completing assigned quality and clinical workflows. The role involves identifying errors and omissions in clinical assessments, OASIS, coding, plans of care, and other clinical documentation. The specialist collaborates with clinicians to ensure accurate documentation and plan of care development, aiming for strong clinical outcomes and participation in quality improvement initiatives.

Requirements

• Registered Nurse with current license from the state of residence.
• HCS-D credentials / ICD10 coding required.
• COS-C credentials/ OASIS strongly preferred or required within 6 months of job entry date.
• Home Care Home Base experience is a plus.
• EPIC experience is a plus.
• Bachelor's degree preferred; Associate degree considered if employed with Trinity Health at Home for 5 or more years and meets performance expectations.
• Home Care or Hospice experience or knowledge required.
• Quality improvement, OASIS review/coding experience required.
• Demonstrated analytical skills required.
• Strong computer and technology skills required.
• Well-developed interpersonal and communication skills necessary.

Nice To Haves

• HCS-O certification is helpful.

Responsibilities

• Collaborate with clinicians to achieve accurate documentation and plan of care development.
• Educate clinicians on quality, outcomes improvement, documentation standards, and actions to improve clinical processes.
• Participate in department quality improvement initiatives.
• Collaborate with the agency on focused quality improvement initiatives.
