About The Position

The Clinical Supervisor is responsible for providing direct supervision of all patient services personnel and case managers. The Clinical Supervisor coordinates all care services and can perform case management functions if needed. The Clinical Supervisor is in compliance with all the accreditation, federal, and state regulations, and reimbursement guidelines as well as Agency policies and procedures. This position reports to the Director of Patient Care Services. Pay: $40/Hour + Mileage The following duties are normal for this position. Other duties may be required and assigned. Directs nursing and home care as prescribed by the attending physician. Reviews all plans of treatments for accuracy, appropriateness, and completeness. Provides high quality leadership, guidance, and supervision to your team, ensuring they understand how to provide compassionate and respectful care and comprehensive case management services to help all patients. Maintains accurate records of all services rendered to patients. Evaluates and regularly reevaluates the care and nursing needs of each patient. Supervises and trains other nursing services personnel. Provides orientation to new personnel as well as competency and performance oversight of existing personnel, as needed. Performs clinical supervisory visits on each Home Health Aide patient every two (2) weeks. Performs initial admission assessment and evaluation of the Agency’s patients. Develops written Plans of Care following each admission. Reports to the physician the patient’s response to treatments, including drugs, and changes in the patient’s physical or emotional condition. Re-evaluates the need for continued care on an ongoing basis and initiates recertification as necessary. Teaches, supervises, and counsels the patient and family members regarding nursing care needs. Supports and carries out the Agency’s policies. Participates in the Agency’s information management and performance improvement programs. Assures that monthly case conferences are done on each patient, recorded, and filed in patient’s chart. Follow up with lab results and notify physicians and care providers of any significant results, as needed. Completes annual education requirements. Assists administration in meeting the accreditation, federal, state, and local regulations and standards. Always maintains and ensures patient confidentiality. Maintains confidentiality of all records and reports maintained for all patients.

Requirements

  • Bachelor’s degree in nursing from an accredited nursing program and current RN licensure in Kentucky with at least two years of nursing practice experience in home care and one year supervisory experience.
  • CPR certification and TB skin test or willingness to receive.
  • Ability to read, analyze, and interpret intake orders, insurance documents, financial reports, and medical orders.
  • Ability to respond to common inquiries from patients.
  • Ability to communicate professionally with other medical professionals, colleagues, and members of the community.
  • Effectively listens to others and provides relevant and timely information to management, colleagues, and patients.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentage.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to adapt and perform job duties in a normal office environment.
  • Primarily sedentary, with the ability to occasionally stand, walk, use hands to handle/feel, reach with hands and arms, stoop, kneel, crouch, crawl, talk, hear, smell, manipulate up to 50 pounds, lift up to 30 pounds of office supplies, and ability to walk up and down a normal flight of stairs.
  • Specific vision abilities include close vision, distance vision, color vision, peripheral vision, and ability to adjust focus.
