About The Position
Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) in Lincoln, Nebraska. In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Lincoln and its surrounding areas. As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes. This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses-including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field.
