About The Position
Acentra Health is hiring Clinical Evaluators (PASRR) to join our growing and dynamic team! As a Clinical Evaluator PASRR on our Pre-Admission Screening and Resident Review (PASRR) team, you will play a vital role in ensuring individuals receive the right care at the right time. By conducting face-to-face assessments in Emporia, Kansas, you'll help facilitate appropriate placements and enhance the quality of life for those you serve. This part-time hybrid role offers the perfect balance of flexibility and purpose. Work from the comfort of your home office while also engaging directly with patients in local settings, such as nursing homes and hospitals. Travel locally to perform essential evaluations, with all travel expenses, including mileage and parking, fully reimbursed. Your work will focus on completing clinical assessments for individuals with serious mental illness (SMI), intellectual disabilities (ID), or related conditions (RC) as they transition to nursing home care. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.
