About The Position

The Clinical Documentation Integrity Specialist at El Camino Health is responsible for enhancing the quality and completeness of clinical documentation across the healthcare system. This role involves collaborating with healthcare providers to ensure accurate documentation that supports appropriate coding and compliance, ultimately improving patient care and outcomes.

Requirements

• Bachelor's degree in Nursing or a medical doctorate degree.
• Minimum of two years of experience as a clinical documentation specialist.
• Five years of acute direct clinical experience.
• Professional Coding Certification (CRC, CCS, CPC) or more than three years of medical coding experience preferred.
• Proficiency in computer usage including database and spreadsheet analysis, presentation programs, and word processing.
• Knowledge of federal, state, and fiscal intermediary regulations.

Nice To Haves

• Certified Clinical Documentation Specialist (CCDS) Credential or Clinical Documentation Integrity Practitioner required.
• Required to obtain Certified Clinical Documentation Specialist - Outpatient (CCDS-O) within 24 months of hire.

Responsibilities

• Provide daily clinical review of medical records, including physician and clinical documentation, lab results, diagnostic information, and treatment plans.
• Review documentation by the Medical Staff and healthcare team in accordance with clinical documentation policies and coding guidelines.
• Communicate with physicians regarding missing or unclear documentation to clarify information and educate on proper documentation practices.
• Educate healthcare team members to ensure accurate and compliant documentation in medical records.
• Demonstrate understanding of clinical complexities, including complications, co-morbidities, and severity of illness, and impart this knowledge to the healthcare team.
• Gather and analyze information pertinent to documentation findings and outcomes.
