Clinical Director of Home Health
About The Position

We’re looking for a Clinical Director to guide our home health team and ensure that every patient receives the highest quality of care, delivered with compassion, clarity, and commitment. As Clinical Director, you’ll oversee patient care services, support our skilled clinical team, and collaborate closely with each department to maintain excellence in care and compliance. Requirements
- Current RN license (required)
- Minimum of one year of recent clinical or management experience (within the last three years)
- Strong clinical judgment, communication, and leadership skills
- Excellent organizational and time management skills
- Proficiency with electronic documentation systems (OASIS experience preferred)
- Home health experience strongly preferred
Responsibilities
- Oversee all patient care services and clinical staff
- Coordinate patient care, assignments, and referrals
- Ensure individualized plans of care are developed and updated
- Provide clinical oversight and leadership to maintain quality and efficiency
- Support agency performance improvement and clinical outcomes initiatives
- Participate in team meetings, supervisory visits, and care conferences
- Manage daily huddles and clinical operations in collaboration with the Executive Director
