About The Position

Requirements

  • Licensed RN with current Oklahoma licensure.
  • Hospice or home care experience (supervisory experience preferred).

Responsibilities

  • Review referrals and assess eligibility in collaboration with the Medical Director.
  • Assign, support, and mentor interdisciplinary clinical staff.
  • Conduct home visits, record reviews, and case conferences to ensure high-quality care.
  • Guide clinicians in establishing therapeutic goals and individualized plans of care.
  • Lead QAPI and performance improvement initiatives.
  • Support hiring, onboarding, and ongoing staff development.
