About The Position

The Clinical Director for the Montgomery County, PA Street Medicine program is a leadership role based out of Pottstown Hospital. This grant-funded position is focused on providing high-quality healthcare to homeless and housing-insecure populations. The Clinical Director, who must be a licensed Nurse Practitioner, will oversee clinical care delivery, manage a team of healthcare providers, and ensure compliance with state and federal regulations.

Responsibilities

  • Develops virtual clinic SOPs.
  • Coordinates care with other providers.
  • Develops Street Medicine Consult SOPs.
  • Supervises staff in the provision of direct clinical services.
  • Monitors employee's progress and completes annual performance reviews within hospital designated time frames.
  • Reviews and monitors clinical records to ensure compliance with professional standards.
  • Assists in programmatic administration and development of services, policies and procedures, compliance needs, Continuing Medical Education Committee and others as appropriate.
  • Documents disciplinary actions and provides coaching to staff as appropriate according to hospital guidelines.
  • Completes initial clinical evaluation.
  • Develops treatment plan.
  • Documents in Epic.
  • Completes discharge summary.
  • Orders and interprets diagnostic and therapeutic tests, relative to patient's age-specific needs; Prescribes appropriate pharmacologic and nonpharmacologic treatment modalities; Monitors the effectiveness of interventions; Collaborates with multidisciplinary team members by making appropriate referrals.
