About The Position

The Director of Collaborative Care will lead the launch, implementation, and expansion of Benchmark Health’s Universal Triage program across partner primary care practices. This leader will play a pivotal role in building the infrastructure for behavioral health integration from designing clinical workflows and staffing models to directly supporting initial patient care and team training. In the early stages, the Director will be actively involved in clinical operations, managing a patient panel to model best practices in measurement-based care, brief behavioral interventions, and coordinated treatment planning with primary care and psychiatric consultants. As the program grows, this role will evolve to focus on scaling the model overseeing Behavioral Health Care Managers who deliver behavioral health interventions within the AIMS Center Collaborative Care framework. The Director will ensure patients receive high-quality, evidence-based support through seamless coordination between care teams, while also developing referral and triage pathways to external mental health services as clinically indicated. In partnership with executive leadership and business development teams, this role will also contribute to expanding Benchmark Health’s partnerships across California and advancing the company’s mission to bring integrated behavioral healthcare to more communities.
