Assumes responsibility for the overall nutrition management of patients. Uses The Nutrition Care Process to assess patients, identify patient-focused nutrition prescriptions and interventions and monitor and evaluate as appropriate. Re-assesses patients based on level of nutrition care complexity and Dept. policy. Works with healthcare team to determine required diet and patient education needs. Provides evidence-based nutrition education to patients, families, staff and the community. Interfaces with Diet Office and tray passing personnel. Performs daily patient rounds; appropriately screens patients based on established guidelines and Dept. policy. Maintains complete and appropriate records. Performs other duties as required.
