Job Summary : Participates in the delivery of inpatient and outpatient care through prompt, courteous and effective assessment of patients' nutritional status, provides patient and community education on normal and therapeutic nutrition, consults with other health professionals as needed. To provide Diabetes Self-Management Education for patients referred to the HRH Center for Diabetes Education. Hendricks Regional Health’s Center for Diabetes Excellence has a recognized diabetes education program. Providers must follow CMS quality standards. We use an inter-professional approach to the provision of Diabetes Self-Management training (DSMT). Job Description Essential Responsibilities: 1. Inpatient Care Responsible for the nutritional counseling for inpatients and patients' families according to physicians' orders. Interacts with physicians, ancillary services and Nursing personnel regarding patients' care. Routinely visits inpatients regarding their food tolerances and preferences and acceptance of their hospital diet. Assists the Director in writing menus, recipe modifications and analysis, and portion control for both cafeteria and patient menus. Conducts Quality Assurance audits and assists in Quality Improvement Process regarding nutritional services. Responsible for assessments and management of and communication regarding patients' nutritional status and needs. Plans and conducts departmental and interdepartmental in-services concerning nutrition. Develops and coordinates nutrition education material for hospital use. Documents inpatient nutritional care according to the established Nutrition Care Process (NCP). Assess malnutrition using established criteria using Nutrition-Focused Physical Assessment. 2. Outpatient Care Responsible for the nutritional counseling for outpatients according to physicians' orders. Develops and coordinates nutrition education material for outpatient setting. Teaches outpatient classes. Documents inpatient nutritional care according to the established Nutrition Care Process (NCP). RD must have the knowledge to bill appropriately for DSMT services. 3. Community Education Develops and present nutrition education programs to the community including health fair participation. Submits nutrition articles on occasion. Responds to community calls or emails concerning nutrition issues. 4. Documentation in the medical record is accurate, clear, and concise using NCP proper terminology and spelling. Available patient and chart information is included and is used to evaluate and assess the patient's nutritional status. Nutrition care plans, actions, and nutritional goals and interventions are in accordance with patient's diagnosis and nutritional problems. Recommendations are assertively pursued by directly contacting the physician when appropriate or flagging the chart to assure communication. 5. Appointments and classes are scheduled to meet client needs and client calls returned in a timely fashion. Tasks within assigned clinical area are completed according to the times delineated in the assignment. 6. Effectively communicates current knowledge and skills in teaching patients and clients in individual or class setting, professional and community groups so that concepts are understood by the audience. Provides the appropriate type and amount of handout material related to the topic, audience and time allotment. Information presented reflects current nutrition knowledge and makes revisions based on appropriate audience feedback. Subject matter is presented in an enthusiastic and positive manner using appropriate interactive techniques and a variety of media. 7. Utilizes work time efficiently to complete responsibilities. Spends 6-7 hours per day on the average in direct patient activities. Prioritizes workload when patient census increases. Effectively uses down time to work on assignments or projects. 8. Participates in hospital-sponsored and other appropriate continuing education opportunities. Reviews dietetic literature to update knowledge and/or increase professional effectiveness and shares information with staff members. Participates in department staff meetings, conducts departmental orientations and in-services to enhance smooth department operation and communication. 9. Promotes teamwork with a courteous, cooperative and positive behavior while interacting with co-workers and customers to ensure that work is completed and runs smoothly, even during periods of peak service times. Helps without complaint when shortages of staff occur. 10. Uses common sense and special medical skills to care for the sick or handicapped with regard to the patient's age and diagnosis. 11. Creates a service culture. 12. DSMT RD Responsibilities Assessment and screening of hospitalized patients with Blood Sugars over 180 or A1cs over 8 Diabetic education for hospitalized patients using DSMT principles Outpatient DSMT Community outreach for diabetic education using DSMT principles All patient/client education uses adult learning principles and includes assessment of barriers, assessment for readiness of change and goal setting. Goal setting is individualized with patient/client and incudes the use of SMART goals and assessment of goal attainment.
