About The Position

The Clinical Dietitian position is responsible for providing in-depth nutrition assessment, counseling, and follow-ups for patients based on the referrals of medical providers. Includes, however, is not limited to: chart review, data collection of biochemical and anthropometric measurements, dietary history, medical history, and physical problem assessments.

Requirements

  • Bachelor's in Science degree in Dietetics, nutrition, and related fields
  • Educational certifications in the area of diabetic health management
  • CPR/First Aid Certification
  • Minimum of two (2) years of experience in a related clinical or hospital environment

Nice To Haves

  • One (1) year of working experience in a medical or physician office setting.
  • Knowledge of The Joint Commission Regulations.
  • Bi-lingual in Spanish, Nepali, or French

Responsibilities

  • Develops, implements, and follows up with a nutritional care plan based on assessment and evaluation of nutritional needs and correlates them with other health care plans.
  • Ensures that all referred prenatal clients are instructed on the importance of weight gain and special dietary needs during pregnancy. This includes plotting patients’ weight gain during pregnancy, assuring that prenatal clients understand the need for special vitamins/minerals during pregnancy, and avoiding drugs and alcohol during pregnancy.
  • Documents pertinent data regarding nutritional care plan, problems, diet order, and evaluation in the patient's medical record, maintaining confidentiality as required.
  • Responsible for providing current nutrition education concepts and procedures to patients. Instruction/education on an appropriate diet (in the absence of a specific diet ordered by a provider).
  • Provide diet education to adults.
  • Provide instruction/education on disease process, for patients including, but not limited to, cardiovascular disease, hypertension, obesity, HIV/AIDS, failure to thrive, gastrointestinal disease, and progressive liver disease.
  • Participates in the interdisciplinary team meeting as an active member and serves as a nutritional consultant. Coordinates with providers, pharmacists, nursing staff, social workers, and other HHC staff when deemed appropriate.
  • Develop and obtain appropriate instructional materials for the scope of care.
  • Performs miscellaneous job-related duties within the general scope of practice as assigned.
