Develops and facilitates the overall training program and orientation of newly hired clinicians and provides intermittent support for existing staff. Additionally works with Education Department for development and oversight of preceptor ship of new nursing hires not participating in the Immersion program.

Requirements

  • Education: Graduate of NLN-approved BSN program required. Master's degree preferred.
  • Experience: Three years clinical experience required. Home care, clinical training and education background preferred.
  • Licensure: CT RN License or CT PT License required. OASIS certification preferred.
  • Excellent verbal and written communication skills, including computer usage and teaching skills.
  • Ability to travel within the region to perform visits, willingness to travel between regions as needed.

Responsibilities

  • Design and oversee individualized education plan that aligns with general orientation matrix for orientees
  • Maintain accurate records of educational interventions/checklists/ folders and follow up with management as appropriate
  • Develop, facilitate communication, and manage schedules for orientees with office staff
  • Demonstrate home visits, skills, and Joint field visits
  • Provide support to orientees with Pre visit prep and review, post visit charting support, and aid with navigating resources and technology
  • Observe and provide feedback of orientees and clinicians based on standard work and chart review
  • Facilitate education rounds with Team Managers and Regional Director
  • Implement and lead 1:1 and group orientee meetings
  • Facilitate the coordination, oversee and provide feedback regarding the interactions of preceptor and clinicians
  • Develop and manage schedules for the preceptor
  • Design individualized education opportunities for staff development and support
  • Assist with ongoing staff development
  • Develop and maintain the updated versions of learning tools/tip sheets on Sharepoint
  • Facilitate regional orientation workshops
  • Maintain personal annual skill and academic competencies
  • Ensures compliance with applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and HHCAH policies and procedures.
  • Abides by and demonstrates H3W Leadership Behaviors and continued process improvement through job performance on a day-to-day basis.
