About The Position

The Data Abstractor is responsible for accurately reviewing and extracting clinical information from medical records to support organizational reporting and compliance efforts. This includes collecting and validating data for programs such as the Uniform Data System (UDS), Centers for Medicare & Medicaid Services (CMS), Accountable Entities (AE), and Patient-Centered Medical Home (PCMH). The role requires close collaboration with clinical, quality, and HIT team to ensure data integrity, support performance improvement initiatives, and meet regulatory requirements.

Requirements

  • Associate's degree in Health Information Management, or related field or Minimum 2 years of experience in medical record abstraction or clinical data reporting.
  • Familiarity with UDS, CMS quality programs, AE reporting frameworks, and PCMH standards.
  • Strong understanding of medical terminology, and clinical workflows.
  • Highly proficient in Microsoft Office applications.
  • Excellent attention to detail, organizational skills, and ability to meet deadlines.

Nice To Haves

  • Working knowledge of NextGen Products preferred.

Responsibilities

  • Extract relevant clinical data from patient charts to support UDS, CMS, AE, and PCMH reporting.
  • Ensure completeness and accuracy of data elements including demographics, diagnoses, procedures, labs, screenings, and care coordination documentation.
  • Conduct audits and validation checks to maintain data accuracy and integrity.
  • Identify and resolve documentation gaps in collaboration with clinical staff.
  • Partner with providers, care teams, and HIT staff to clarify documentation and improve data capture.
  • Participate in meetings focused on quality improvement, reporting timelines, and workflow optimization.
  • Utilize EHR systems (e.g., NextGen, Epic, ESCL, Current Care, Kidsnet) and reporting platforms to locate and abstract required data.
  • Maintain proficiency in abstraction tools and templates.
  • Support outreach efforts by identifying patients with gaps in care and coordinating follow-up actions with care team.
