About The Position

Make a difference in the lives of patients with chronic kidney disease. The Clinical Coordinator RN will work on-site at DaVita's 6-chair Skilled Nursing Facility dialysis dens. The modality eliminates the need for patient travel to an outpatient dialysis facility. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a burden on patients and adding avoidable costs to the health care system. The home site is DaVita SNF Dialysis Fenton 512 Beach St, Fenton, MI 48430. DaVita is seeking a Clinical Coordinator RN to lead dialysis care in a skilled nursing facility. In this role, you’ll oversee patient care delivery, support a clinical team, and ensure treatment meets the highest standards of safety and quality. Requirements

  • RN license in the state of practice
  • Minimum of 18 months of RN experience
  • Minimum 6 months of dialysis experience
  • CPR certification
  • Charge RN readiness approval required
  • Proficiency in MS Word and Outlook

Nice To Haves

  • CNN or CDN certification preferred
  • Supervisory experience preferred

Responsibilities

  • Coordinate care for ESRD patients, including labs, vitals, and outcomes
  • Lead and support a team of clinicians and PCTs
  • Deliver direct patient care in a skilled nursing facility
  • Monitor patient status and adjust care as needed
  • Ensure compliance with all clinical and regulatory standards
  • Support Administrative Growth
  • Communication with our SNF partner
  • Coaching teammates on the resolution of issues and escalating to Regional Operations Manager as appropriate.
  • Training teammates to include in-services
  • Managing scheduling and call-ins
  • Staffing the floor ~50% of the time
