About The Position

The Clinical Coordinator (Post Acute and Population Health), will support staff in coordinating and managing care for Endeavor patients across post-acute and ambulatory settings. This role focuses on optimizing clinical outcomes, resource utilization, and patient care coordination across various care settings and providers. The Clinical Coordinator will assist in ensuring the delivery of high-quality care and service excellence, while contributing to the effective management of clinical resources. They will collaborate with physicians, clinical leaders, and interdisciplinary teams to ensure seamless care transitions and continuity for patients. Additionally, the Clinical Coordinator will provide support in the operational and administrative functions of care coordination across the post-acute network. This position requires strong leadership skills, clinical expertise, and a commitment to excellence in healthcare.

Requirements

  • Bachelors Degree Required
  • Masters Degree Preferred
  • Clinical background including nursing, social work, or another clinically-related healthcare field or similar advanced degree required. And 3 Years Minimum 3 years of quality management/care coordination/health care experience
  • Strategic thinker with experience in a value-based care management environment; Care Management/Utilization Review experience with CMSA or ACMA certification preferred.
  • Experience working with physician colleagues to build and advance clinical programs
  • Strong interpersonal, oral and written communication skills; able to navigate EMR and Microsoft Office Suite of applications
  • Strong leadership, change management and project management and presentation skills
  • Experience in data management and using data to inform decision-making
  • BLS – Basic Life Support (CPR and AED) - American Heart Association (AHA) Preferred
  • Holds an active unencumbered license to practice in their discipline of Nursing, Rehab, or other related clinical discipline Required
  • Clinical certification, such as case management certification, is preferred Preferred
