About The Position

The Clinical Coordinator RN will operate in direct partnership with IMG practices. To assist our patients in navigating the variousplexities of our health system and act as a resource to coordinate the patients' needs with ouranization resources. This role will act as a clinical support for the IMG practices by support their pool's and provide clinical triage. This role willplete triage transfers by utilize protocols and clinical judgement to assess and prioritize a patients need and determine the appropriate course of action.

Requirements

• Associate degree required.
• Bachelor's degree preferred.
• Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare.
• Experience in a care management role preferred.
• Experience in Ambulatory care preferred.
• 3-5 years of direct patient care experience required.
• Active New Jersey Registered Nurse License is required.
• Active Basic Life Support (BLS) certification is required.
