About The Position

The Clinical Coordinator RN will operate in direct partnership with IMG practices. To assist our patients in navigating the various complexities of our health system and act as a resource to coordinate the patients' needs with our organization resources. This role will act as a clinical support for the IMG practices by support their pool's and provide clinical triage. This role will complete triage transfers by utilize protocols and clinical judgement to assess and prioritize a patients need and determine the appropriate course of action.

Requirements

• Associate degree required.
• Active New Jersey Registered Nurse License is required.
• Active Basic Life Support (BLS) certification is required.
• 3-5 years of direct patient care experience required.

Nice To Haves

• Bachelor’s degree preferred.
• Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare.
• Experience in a care management role preferred.
• Experience in Ambulatory care preferred.
