Provides an optimum environment for positive communication and health enhancement for orthopedic patients through the continuum of care and coordinates services for the Ortho Joint program. Utilizes interdisciplinary collaboration with the Health Care Team and available resources to enhance coordinated care for the program and program participants. Plans, leads and implements activities to enhance access to and quality of Orthopedic Services relative to services and programs. Responsible for the development and maintenance of accurate, timely and compliant rehab and services and activities.

Requirements

  • Minimum of one of experience as a Program/Clinical Coordination in a healthcare field.
  • Bachelor`s Degree in Health Sciences.

Nice To Haves

  • Prior experience supporting Orthopedic Joint programs, Orthopedic Nursing, Rehabilitative Services.
  • Master`s Degree in Healthcare discipline.
  • RN, APN, PA, PT, OT.

Responsibilities

  • Identifies areas requiring clinical study relative to the joint program for performance improvement action.
  • Demonstrates the ability to function as a clinical resource person for the Joint Program through offering professional development opportunities.
  • Regularly incorporates new evidence - based practices into orthopedic educational programs and utilizes information to further enhance the quality of patient care through the joint program.
  • Communicates in oral/written format to Nursing Administration/Managers and Rehabilitation leadership the scope of orthopedic clinical services planned/accomplished, both as requested and as established on a monthly basis.
  • Communicates (orally, written) effectively with physicians, physician offices, patients/families & healthcare team.
  • Effectively engages patients/families and health care team in clinical service line programs
  • Plans, implements and promotes clinical service line initiatives/educational programs.
  • Organizes and facilitates patient teaching and support programs for patients/families re: orthopedic joint program services.
  • Utilizes appropriate teaching principles, strategies and techniques when educating patients/health care team re: orthopedic service line.
  • Collaborates with case managers, assisting in facilitation appropriate coverage/justification of services to ensure that patient needs are met, and patients flow through the continuum of care is seamless.
  • Demonstrates resource capability through use of other disciplines within the continuum of care.
  • Coordinates and assists with preparation on special projects as requested by Leader. 13. Retrieves and utilizes pertinent data from the clinical financial and performance measurement information systems. Analyzes performance measurement data identifies patterns and trends for improvement and follow up as needed.
  • Maintains an organized system for quality performance improvement review activities. Uses internal and external benchmark processes to evaluate performance. Collects patient care data relative to issues and prepares summaries of review activities and develops monitoring activities as needed. Participates in performance improvement Educational programs.Coordinates efforts with PI steering QI and outcomes committees 15. Participate in regularly scheduled meetings and provide regular reports to update participant progress and identify potential and actual barriers to success.
  • Designs, plans, implements enhancements, coordinates upgrades and maintains security of Joint Program related materials and records. Designs, plans, coordinates and implements enhancements to rehabilitation documentation for medical records.
  • Facilitates interdisciplinary internal work groups to make positive changes with measurable outcomes and effectively communicates findings. Applies the theories of Performance Improvement including statistical data to all PI activities.
  • Participates in the planning and preparation activities for regulatory agency surveys. Promotes the understanding of compliance with current regulatory agency standards / requirements. Serves as a resource during the surveys with regard to compliance with all standards and requirements.
  • Develops, supports and implements projects that enhance departmental performance. Collaborates with managers and other departments in implementing strategies to ensure success.
  • Coordinates the standards set forth by regulatory agencies, as applicable including Joint Commission, NJ Department of Health etc.
