About The Position

We’re looking for a Licensed Practical Nurse (LPN) to serve as our Clinical Coordinator to join our growing team and become a key part of our healthy, respectful, and fun team culture. Requirements

  • Education: Graduate of an approved school of practical (vocational) nursing
  • Active LPN license in the state of Massachusetts
  • Experience: Minimum 1 year of home health experience preferred
  • Current BLS certification
  • Tech-Savvy: Comfortable using Microsoft Office and electronic health records (EHR); experience with systems like KanTime, Axxess, WellSky, or HCHB is a plus
  • Clinically grounded and administratively sharp
  • Calm under pressure and great with people
  • Motivated by building something meaningful
  • Comfortable wearing multiple hats and working in a fast-paced, early-stage environment
  • Strong communicator and team motivator
  • Positive, solution-oriented mindset with strong attention to detail
  • Strong organizational skills: knows how to juggle tasks, manage priorities, and keep things moving efficiently

Responsibilities

  • Coordinate intake of new referrals, including reviewing clinical information and ensuring appropriateness for home health services
  • Communicate with referral sources, patients, and families to facilitate smooth admissions
  • Verify patient insurance eligibility and obtain necessary authorizations to ensure smooth reimbursement processes
  • Collaborate with the RN Clinical Manager to ensure timely starts of care: coordinate patient visits and align care needs with staff availability
  • Support daily office operations related to scheduling, intake, orders and documentation
  • Serve as a central point of contact between clinicians, patients, referral partners, and payers
  • Support Compliance: Maintain accurate records in the electronic medical record (EMR) and stay on top of ever-changing Medicare, Medicaid, state regulations, and agency requirements
  • Assist with onboarding coordination, payroll support, and basic administrative workflows as needed
  • Provide on-call support on a rotational or as needed basis
  • Provide intermittent skilled nursing support in the field under the direction of the RN, based on agency needs
  • Implement nursing care plans in accordance with physician orders and agency policy
  • Observe and report patient status changes to the Clinical Manager, RN and/or physician
  • Educate patients and families on care needs and treatment plans
