The Clinical Coordinator is responsible for maintaining clinical practice standards, utilizing and promoting evidence based practice and guiding the VNA interdisciplinary team. Acts as a staff resource and liaison to facilitate staff development. Works collaboratively with Patient Care Manager to maintain standards of professional nursing practice.

Requirements

  • Graduate of an accredited professional nursing program.
  • Registered Nurse with current license to practice in the state of Pennsylvania or seeking licensure through endorsement.
  • Two years experience in home health or hospice.
  • Evidence of successful completion of BLS.

Responsibilities

  • Demonstrates expertise in areas of clinical practice.
  • Supports Patient care Manager in maintaining and supervising daily operations, clinical oversight, training and competency.
  • Assists Patient Care Manager in developing, evaluating and adjusting current and future staffing based upon patient care needs; initiates adjustments as needed.
  • Uses Nursing Process as framework for patient care delivery.
  • Acts as a liaison between interdisciplinary team to facilitate optimal case management and discharge planning.
  • Assigns responsibility for patient care.
  • Assists with staff education activities (i.e. orientation, competency, skill reviews, mandatory in servicing, and evaluation of student clinical placements.)
  • Provides input to annual performance reviews of assigned staff.
  • Maintains awareness of unit budget.
  • Consistently fosters a positive work environment.
  • Takes active role in program performance improvement initiatives.
  • Maintains responsibility for own professional development through active participation in in-services and/or continuing education programs.
  • Participates in identifying departmental needs and supports departmental goals and objectives that contribute to the growth of the Program.
  • Functions as a professional role model and resource person providing guidance to coworkers.
  • Complies with established chain of command for work-related problems.
  • Assists with maintenance of departmental records for administrative and regulatory issues.
  • Maintains confidentiality of all materials handled within the Network/Entity as well as the proper release of information
  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.
  • Demonstrates/models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).
  • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.
  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
  • Complies with Network and departmental policies regarding attendance and dress code.
  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated
