About The Position

The Clinical Case Manager plays a vital role in supporting the treatment process by coordinating care for clients and families, strengthening communication across the treatment team, and ensuring that each client’s treatment journey is organized, supported, and aligned with clinical goals. Working closely with therapists, psychiatrists, families, and external providers, the Clinical Case Manager helps ensure that clients receive comprehensive, coordinated care that supports both clinical progress and long-term stability. The Clinical Case Manager reports to the PROGRAM SUPERVISOR.

Requirements

  • Bachelor’s or Master’s degree in psychology, social work, counseling, or a related behavioral health field.
  • Experience working in mental health, behavioral health, or substance use treatment settings preferred.
  • Strong organizational and communication skills.
  • Ability to collaborate effectively with multidisciplinary clinical teams.
  • Experience coordinating care with families and external providers preferred.
  • Knowledge of mental health treatment systems, levels of care, and community resources preferred.
  • Must have valid California driver’s license and liability insurance if driving personal and/or company vehicle on Balance Treatment Center business.
  • Vision, hearing, manual dexterity and hand-eye coordination must be adequate for performance of job duties. Able to sit at desk, use keyboard, write and physically perform other job duties. Able to move about the facility to observe clients and staff.
  • Must be customer-service driven.
  • Personal vehicle with liability insurance for reimbursable mileage, generally short distances.
  • Class “C” Driver’s License.
  • First Aid certification, obtain within 90 days of employment and maintain current thereafter.
  • CPR certification, obtain within 90 days of employment and maintain current thereafter.
  • Medication training required within 30 days of employment.
  • Develop computer skills adequate to perform word processing duties within 90 days of employment.

Responsibilities

  • Maintain the well-being of clients and the integrity of the program.
  • Coordinate care between therapists, psychiatrists, families, schools, and external providers to ensure continuity of treatment.
  • Serve as a primary point of contact for families regarding care coordination and treatment logistics.
  • Support communication between Balance Treatment Center and referring providers, outpatient therapists, and other community partners.
  • Facilitate coordination of psychiatric appointments, outside providers, and additional clinical resources when appropriate.
  • Participate in multidisciplinary treatment team meetings to review client progress and clinical needs.
  • Support therapists in tracking treatment plan goals and client progress.
  • Assist in updating treatment plans and documenting coordination of care.
  • Ensure treatment planning reflects collaboration with clients and families whenever appropriate.
  • Maintain consistent communication with families regarding treatment progress, coordination needs, and discharge planning.
  • Provide psychoeducation to families about the treatment process and available resources.
  • Support families in navigating challenges that may arise during treatment participation.
  • Begin discharge planning early in treatment to ensure continuity of care.
  • Coordinate referrals to outpatient providers, step-down programs, or additional services when clinically appropriate.
  • Ensure follow-up appointments and treatment connections are established prior to discharge.
  • Maintain accurate and timely documentation of care coordination in the EMR.
  • Ensure documentation meets clinical, regulatory, and payer requirements.
  • Support therapists and program leadership in maintaining organized clinical records related to care coordination and discharge planning.
  • Work collaboratively with therapists, psychiatrists, program leadership, and administrative staff to support the treatment environment.
  • Participate in team meetings and case consultations.
  • Assist in identifying barriers to treatment engagement and developing solutions with the clinical team.
  • Assist with coordination related to client scheduling, treatment participation, and program logistics when needed.
  • Help maintain a structured and organized treatment experience for clients and families.
  • Support the program in maintaining strong relationships with referral sources and community providers.
