The AT HOME Clinical Case Manager supports the multidisciplinary permanent supportive housing team with clinical skills and experience. The primary role of the Clinical Case Managers is to assess, plan, develop, coordinate, and provide therapeutic and rehabilitative services to an average case load of 17 clients with severe and persistent mental illnesses, chronic health conditions, and/or substance use disorder. The AT HOME Clinical Case Manager will utilize Housing First, harm reduction, trauma informed approach to assist clients in overall improved well-being, stability, and quality of life in conjunction with safe and stable housing. The position provides services in clients’ homes at a single supportive housing property and assists them in accessing additional services, resources, and supports in the community.
