Our organization offers 50+ programs that address mental health, the unhoused, early care and education, those impacted by the justice system, transitional age youth, as well as aging adults. Our programs combine the latest scientific research with cultural sensitivity and a deep commitment to supporting and reflecting the communities we serve. Serving adults living with serious mental illness, the Adult ICM program provides an extensive array of recovery-oriented, age-appropriate services and supports, including linkage to housing and basic needs, physical health care, benefits assistance, and employment services, and integrated mental health and substance abuse treatment services. The programs follow an assertive community treatment model, providing medication management, individual and group therapy, individual and group rehabilitation skills training, crisis intervention, and case management. The programs consist of multi-disciplinary teams of a psychiatrist(s), nurse practitioner(s), licensed vocational nurse(s), a program director, clinical case managers, case managers, a client support specialist, and an administrative manager. Services are provided to clients both in the office and in the field. The Clinical Case Manager meets with and evaluates the needs of clients and develops a treatment plan in collaboration with the client. The Clinical Case Manager provides specialized mental health services, including therapy, rehabilitative skills training, crisis intervention, and case management. The Clinical Case Manager provides a safe and supportive environment for the client and their support system and adheres to laws regarding confidentiality and mandated reporting. Services are provided both in the field (hospital, client’s home, courthouse, doctor’s office, etc.) and in the office. The Clinical Case Manager must maintain accurate detailed records in our electronic health record system. The caseload for each CCM is about 17 clients.
