Serving adults living with serious mental illness, the Adult ICM program provides an extensive array of recovery-oriented, age-appropriate services and supports, including linkage to housing and basic needs, physical health care, benefits assistance, and employment services, and integrated mental health and substance abuse treatment services. The programs follow an assertive community treatment model, providing medication management, individual and group therapy, individual and group rehabilitation skills training, crisis intervention, and case management. The programs consist of multi-disciplinary teams of a psychiatrist(s), nurse practitioner(s), licensed vocational nurse(s), a program director, clinical case managers, case managers, a client support specialist, and an administrative manager. Services are provided to clients both in the office and in the field. The Clinical Case Manager meets with and evaluates the needs of clients and develops a treatment plan in collaboration with the client. The Clinical Case Manager provides specialized mental health services, including therapy, rehabilitative skills training, crisis intervention, and case management. The Clinical Case Manager provides a safe and supportive environment for the client and their support system and adheres to laws regarding confidentiality and mandated reporting. Services are provided both in the field (hospital, client’s home, courthouse, doctor’s office, etc.) and in the office. The Clinical Case Manager must maintain accurate detailed records in our electronic health record system. The caseload for each CCM is about 17 clients.

Requirements

  • Master’s degree in Social Work, Psychology or related fields of study required.
  • Must have experience in case management and mental health services with severely mentally ill, dually diagnosed adults.
  • Comprehensive knowledge of mental health and substance abuse counseling, assessment, and crisis intervention skills.
  • Must be, or have the potential to be, registered with the BBS.
  • Willingness to work from a strength-based, harm reduction, client-centered model with an emphasis on evidence-based practices.
  • Ability to work cooperatively on a multi-disciplinary team, and to work with people of various ethnic and cultural backgrounds.
  • Strong organizational, communication, documentation, time management, and interpersonal skills.

Responsibilities

  • Provide individual and group therapy, individual and group rehabilitative skills training, case management, and crisis intervention services to our clients.
  • Link clients to basic needs, including income, housing, job training, insurance, and/or nutrition, among other resources.
  • Assist clients in creating and maintaining stability in the community by offering at least weekly appointments.
  • Collaborate with community partners in housing, finance, basic needs, and medical services.
  • Provide independent living skills education to clients in the office and in their home.
  • Responsible for an average of 60-66 hours of direct services per month.
  • Maintain timely documentation of services provided that meet Medi-Cal standards.
  • Provide officer-of-the-day responsibilities to TAY and Adult Mental Health Program clients.
