About The Position

The incumbent is responsible for providing case management services to individuals with mental illness(es) and/or substance use disorders. Emphasis will be placed on case management services for the SMI, SED, and ATR populations. This includes clinical assessment, treatment planning, discharge planning, linking with community services, aggressive outreach and follow-up. This position is directly responsible to the Senior Case Manager and Coordinator of Clinical Case Management Services and is supervised by the Coordinator of Clinical Case Management Services. The position is exempt for FLSA. The position is designated as a Level Two with regard to the HIPAA Privacy Rule. OSHA regulations apply to this position.

Requirements

  • This position requires a Bachelor's Degree in Human Services with preference given to applicants with a minimum of two years of related experience.
  • The ideal incumbents must also possess the knowledge, skills, and qualifications in the Case Management Licensing Regulations to fulfill the job requirements.
  • A valid driver's license and an acceptable driving record is also required.
  • This position will be providing case management services to adults with mental health illness and /or substance abuse.
  • Emphasis will be placed on case management services.
  • The position is located in Lunenburg, and Farmville, Virginia.

Responsibilities

  • Responsible for assessment and identification of case management service needs.
  • Responsible for supportive counseling to individuals and families to keep stable community functioning.
  • Responsible for development of treatment planning and coordination with other service providers.
  • Responsible for follow-up and outreach to individuals in order to maintain active treatment participation. This includes home visits to monitor functioning and provide outreach as per agency protocol.
  • Responsible for maintaining Electronic Health Record and as per agency policies and procedures.
  • Responsible for coordination with RN/LPN, NP and psychiatrist for monitoring efficacy of psychotropic medications and coordinating in response to emergent psychiatric decompensation.
  • Responsible for maintaining contact for the first thirty days with private and state hospitals to ensure treatment planning, discharge planning and coordination of individual’s care as per state and agency protocol and procedures.
  • Responsible for advocating, linking and coordinating with service providers and other community resources for the purpose of arranging necessary services. This includes assisting the individual or family member in the application process for benefits.
  • Responsible for the provision of transportation for individuals to ensure continuity of services.
  • Responsible for attending in-service training and staff development programs to enhance case management services.
  • Responsible for other duties as assigned and consistent with job description and expectations for the position.
  • Responsible for attending weekly staff meetings and participating in case discussions as a member of the clinical team.
