Under supervision of the Program Director, the clinical case manager participates in a multidisciplinary team, including external and internal agency team members consisting of clinicians, primary care providers, nurses, peer specialists, interns, and psychiatric medication providers, serving as the lead point of responsibility for enacting onsite clinical client care. The Knox has 30 open units which will become occupied in the next few months with clients to be served within this program. The tenants occupying the additional units in the building will not be part of the clinician’s caseload but could require intervention from Felton staff. The caseload is capped at a lower level to allow for intensive services to stabilize individuals who experience acute symptoms and related risk factors. The clinician evaluates the needs of clients, and develops comprehensive clinical assessments, treatment plans, and coordinates care among the team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, and engagement strategies. The clinician maintains accurate, detailed records of client contacts through Cal-AIM (Medi-Cal) and Medicare standards as well as county and state regulations. The clinician will work 60% onsite at The Knox and 40% in other Senior Division programs including other supportive housing sites. Additional duties as needed.
