About The Position

Under supervision of the Program Director, the clinical case manager participates in a multidisciplinary team, including external and internal agency team members consisting of clinicians, primary care providers, nurses, peer specialists, interns, and psychiatric medication providers, serving as the lead point of responsibility for enacting onsite clinical client care. The Knox has 30 open units which will become occupied in the next few months with clients to be served within this program. The tenants occupying the additional units in the building will not be part of the clinician’s caseload but could require intervention from Felton staff. The caseload is capped at a lower level to allow for intensive services to stabilize individuals who experience acute symptoms and related risk factors. The clinician evaluates the needs of clients, and develops comprehensive clinical assessments, treatment plans, and coordinates care among the team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, and engagement strategies. The clinician maintains accurate, detailed records of client contacts through Cal-AIM (Medi-Cal) and Medicare standards as well as county and state regulations. The clinician will work 60% onsite at The Knox and 40% in other Senior Division programs including other supportive housing sites. Additional duties as needed.

Requirements

Master's degree or Licensed in Social Work; Marriage and Family Therapy, or Professional Counseling and registered with an active license with CA BBS, including ASWs, AMFTs, and APCCs.
A minimum of 2 years of case management and mental health treatment for at-risk populations, including co-occurring disorders (master’s level internship experience considered).
Experience working with unhoused or formerly unhoused community members, with a strong grasp on housing needs and potential barriers to maintaining stable housing.
Experience managing crisis situations, skilled at crisis response, crisis de-escalation, and risk mitigation.
Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care services definitions, in addition to skills in assessment, treatment planning, and mental health services pertaining to individuals living with serious mental illness.
Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5 and other measures including social determinants of health and functional impairment screenings where appropriate
Demonstrated outreach, rapport building, and engagement experience.
Ability to travel in SF and work in office, community, home, and hospital settings.
Strong skills in working independently and also as a team member.
Strong organizational, communication, interpersonal, and time management skills and ability to prioritize projects and follow through.
Problem solving skills: able to discern when to consult, when to make independent decisions, and able to offer suggestions for solutions.
Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models.
A strong understanding of harm reduction in supportive housing.
Actively practices cultural humility and promotes a warm and welcoming environment for culturally and gender diverse individuals.
Openness to learn, consult, and receive constructive feedback.
Available to work business hours, Monday through Friday, predominantly in field/office settings. Must be comfortable being flexible across worksites.
Excellent communication skills both in written and verbal formats.
Demonstrated computer skills (Microsoft Office, Mac and PC OS) and ability to learn electronic documentation systems.
Driver’s license preferred.

Responsibilities

Work closely with interdisciplinary team.
Perform intakes, assessments, and service plan development for adults with psychiatric and dual diagnosis disabilities.
Provide clinical case management, which includes individual or group therapy, case management, connection to primary care, and coordination with other providers.
Maintains clinical records of each client contact or service in accordance with mandated standards of Medi-Cal, Medicare, and County Behavioral Health Services, and meets requirements of timely access for client care, documentation, and billing requirements.
Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
Provide crisis intervention to prevent hospitalization, and if necessary, facilitates involuntary hospitalization.
Address specific housing needs via training, coaching, or community resource referrals to support clients in maintaining stable housing.
Commit to ongoing training, including developing specializations in evidence-based practices.
Use the ONE system to maintain client records related to housing by HSH standards.
Deliver exceptional customer service to clients, community partners, and team.
