Summary: The Women's Recovery Services Clinical Case Manager is responsible for providing individual case management and facilitating group counseling for program residents. The Clinical Case Manager will maintain a case load of up to nine clients. Residents of the program are primarily dually diagnosed with mental health and substance use disorder. Schedule: Tuesday-Saturday 8:00am-4:00pm (40 hours per week)

Requirements

  • High school diploma or equivalent with 2 years of working experience in related field or
  • Associates degree in related field with a minimum of 1 year experience
  • A valid US driver's license and one year of driving experience is required.
  • Strong written and verbal communication skills
  • Ability to maintain accurate and organized records
  • Ability to respond effectively in crisis situations
  • Ability to work independently and collaboratively as part of a team.
  • Ability to be certified and maintain such certification, in PBS, PABC Tier 1, First Aid/CPR and complete all other mandatory training requirements based on Bridgewell policies and the program's funding sources.
  • May need to transport individuals based on program needs.
  • Requires acceptable CORI results according to Bridgewell policy.

Nice To Haves

  • Individual and group counseling experience preferred.

Responsibilities

  • Complete Individualized Action Plans within first 2 weeks of admission and update them as clinically indicated.
  • Be proficient in the use of the agency electronic health record (EHR)
  • Meet weekly with assigned individuals to support ongoing treatment needs.
  • Actively participate in clinical supervision weekly with clinical director
  • Ensure all documentation in the Medical Record adheres to established service line procedures.
  • Continue to expand professional knowledge and participate in required and recommended trainings.
  • Maintain an understanding of DPH regulations and CARF standards, as applicable to the role.
  • Establish and maintain communication with DCF, guardians, medical providers, therapists, and other relevant parties to support coordinated care.
  • Make appropriate external referrals as clinically necessary.
  • Attend weekly staff meetings and clinical meetings as scheduled.
  • Provide weekend, holiday, and emergency coverage as assigned.
  • Transport clients, using the program van to meetings, appointments, social/recreational events when requested.
  • Ensure safe operations of the program by providing safety training per the Safety Calendar, addressing program physical maintenance needs, conducting audits as required, and alerting management to any concerns.
  • Demonstrate the core philosophy of universal supports by embracing Bridgewell's core values: dignity, respect, caring, integrity, safety and wellness
  • Ensure the confidentiality of Personal Health Information, records of people served, and other related information is in compliance with HIPAA, and other state or federal mandates.
  • Perform other duties as assigned.
