About The Position

The UCC Clinical Case Manager works closely with the Associate Director of the RESET Team (Rapid Evaluation, Stabilization, and Engagement for Treatment) in providing case management services to students, and supporting UCC staff through planning, implementing, and coordinating resources based on individual student and staff needs. The goal of this position is to promote continuity of care through coordinating, collaborating and advocating based on students' needs; coordinating care with students receiving services from other campus resources within and outside the Student Care Network as well as community resources including Vanderbilt University Medical Center (VUMC); assists UCC staff with the hospitalization process, discharge planning, follow-ups, and coordination of other services; establishes, maintains and coordinates relationships with campus and external resources that would assist with the students' needs; consults with other staff to discuss resources; consults/meets with students to determine their needs and best course of treatment; provides transitional support to referring therapist and clients in need of referral to community services; acts as liaison and coordinates between Student Care Coordination (SCC), UCC and community referral sources, including hospitals, private practitioners, clinics, psychiatry, and other mental health agencies while maintaining a shared database of community referral resources in partnership with the SCC.

Requirements

  • A master's degree from an accredited institution of higher education in counseling, social work or other mental health-related field.
  • Licensure is strongly preferred.
  • Eligibility for licensure in Tennessee within two years of employment.

Responsibilities

  • Track no-shows/cancellations of high-risk clients.
  • Schedule off-campus care referrals requested by UCC clinicians, including obtaining authorization to release and obtain client information and/or medical/psychotherapy records.
  • Develop effective working relationships with the Vanderbilt community, including Student Care Network entities, academic deans, faculty, residential staff, Dean of Students departments, Vanderbilt University Police Department, and other relevant campus partners.
  • Assist other Student Affairs offices with coordination of care as needed.
  • Monitor and update UCC Clinicians of midterm deficiencies, academic dismissals or probations of Students on their caseloads with the academic calendar.
  • Participate in weekly Counseling Services meetings with other Student Affairs offices to provide clinical updates and insight.
  • Provide outreach and educational programming to campus community to support campus and individual mental health, wellness, and skill development.
  • Maintain appropriate and timely documentation for recordkeeping and communication with other service providers.
  • Work collaboratively with other case managers at the UCC to support students and coordinate care with campus partners.
  • Engage in urgent care support and follow-ups for students seen in urgent care of a higher acuity.
