About The Position

Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and recidivism. Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. The Doe Fund’s Scatter Site Return Housing Program (SSR) focuses on assisting chronically homeless individuals in their transition from shelters to community living. The services include relapse prevention and crisis intervention using the principles of Harm Reduction and Motivational Interviewing, education on self-advocacy, community integration, safety, money management, socialization, employment skills, and referrals to appropriate treatment facilities. Through the Scatter Site supportive housing program, clients are moved into apartments located in the Bronx, Brooklyn, and Manhattan areas. We are currently seeking a Clinical Case Manager. The person in this role will be responsible for providing supportive case management services including relapse prevention and recovery readiness to the Scatter Site Return clients.

Requirements

  • You have earned a bachelor’s degree in Social Work or a related field.
  • You have experience working with the homeless, substance abuse, and/or mental health issues.
  • You have working knowledge of DSM IV-TR helpful.
  • Must have knowledge of MS Office (Excel, Word, PowerPoint).
  • Must have knowledge of MS Outlook.

Responsibilities

  • Participate in the intake process for all new clients by completing a mental health assessment.
  • Develop and facilitate support groups within the program for clients.
  • Provide supportive counseling and crisis intervention around a host of issues integral to living with mental illness/ substance abuse issues to clients and/or their families/significant others.
  • Completes psycho-social evaluations for all clients.
  • Serves as primary client contact to foster participation in the program.
  • Documents client contacts and maintains a confidential record.
  • Establishes and maintains weekly meetings with clients based on assessment of client needs.
  • Completes assessment tools required by DOHMH and periodic service plans.
  • Provides counseling and crisis interventions as necessary.
  • Assist the client with adjustment to the apartment through ongoing meetings and life skills training.
  • Orients clients about tenant rights and responsibilities in maintaining and retaining housing.
  • Other duties as assigned to meet programmatic and client needs.
