About The Position

The Clinical Case Manager II in the Neo-Natal ICU (NICU) is responsible for utilizing professional nursing skills to assess patient and family scenarios, ensuring clinical appropriateness for the setting. This role involves establishing effective discharge plans for infants by integrating and providing services across the healthcare continuum, acting as an advocate for patients, families, and significant others.

Requirements

  • Bachelor of Science in Nursing (BSN) degree from an accredited school of nursing.
  • Minimum of three (3) years of case management or utilization review experience in a hospital setting.
  • Minimum of three (3) years of level III NICU experience.
  • NRP (Neonatal Resuscitation Program) certification in accordance with the NRP curriculum.
  • Maintain any neonatal certification obtained during employment.
  • Basic Life Support (BLS) Instructor certification within ninety (90) days of hire/transfer.
  • Previous experience with hospital information systems (order entry, results reporting, case management).
  • Proficient experience using PC/computer (word processing at a minimum).

Nice To Haves

  • Solid clinical assessment, critical thinking, decision making, and organizational skills.
  • Active Case Management Certification (CCM or ACM-RN) or within one (1) year of hire.
  • Strong ability to communicate effectively both verbally and in written work.
  • Strong interpersonal skills and ability to work collaboratively with leaders, staff, patients, families, healthcare team, payers, and external agencies.
  • Demonstrated ability to manage multiple tasks and adjust priorities according to patient needs.
