Clinical Case Managers provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each person we serve’s abilities to function in the community. This role involves assessing, monitoring, and following up with individuals' ability to meet basic needs in moving toward recovery, and is responsible for all clinical documentation of consumer interaction. The position also requires providing transportation to people served in the employee's personal vehicle, with insurance costs reimbursed.
