About The Position
The Clinical Case Manager works as part of the Residential Services team to provide direct support to formerly homeless individuals who have transitioned into permanent housing. They operate from a low-barrier, housing first approach, to ensure residents develop skills and have access to resources they need to live increase their stabilization and maintain permanent housing. The Clinical Case Manager works as part of a stabilization team to provide wrap around case management services, crisis intervention, triage and clinical consultation to stabilize residents who are currently or previously used substances, have severe mental illness, and/or have complex medical conditions. The Clinical Case Manager works collaboratively with other departments at St. Francis House to help stabilize residents in behavioral health, income, and housing.
