About The Position

This application is an evergreen opportunity, which means by applying once, you’ll be considered for current and future Clinical Case Manager openings at our Linden Oaks Hospital in Naperville on our Geropsych unit. The position involves providing quality clinical therapeutic services to patient populations and their families/significant others. As a member of the interdisciplinary team, you will support the organization's treatment program and philosophy, ensuring the delivery of quality treatment. You will also orient and mentor new staff to the unit and provide clinical supervision to Master’s level interns.

Requirements

  • Master’s Degree in Social Work or a Master’s Degree in a recognized mental health field.
  • Possesses the skill set to supervise Master’s level interns.
  • Knowledge of developmental psychology, disease pathology and clinical needs of assigned population.
  • Ability to maintain information which is highly confidential.
  • Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation.
  • Ability to demonstrate self-control in volatile or hostile situations.
  • Ability to set therapeutic limits and maintain therapeutic distance with clients.

Nice To Haves

  • Two years of experience in a behavioral health setting.

Responsibilities

  • Coaches and mentors new hires.
  • Educates new employees on unit to clinical skills related to development of care plans, clinical documentation and understanding group process.
  • Provides clinical supervision to master’s level interns as designated by the unit leader.
  • Monitors evaluation process of students to ensure accurate assessments of skills.
  • Maintains professional relationship with the identified advisor at the student’s academic institution.
  • Actively participates on committees and councils as assigned.
  • Develops and delivers community education programs and workshops to enhance community awareness of mental health issues and resources available to address these issues.
  • Conducts psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.
  • Contacts appropriate previous and current tertiary caregivers to obtain relevant collateral clinical data needed for the patient’s coordination of care, treatment planning and discharge planning.
  • Develops, implements and monitors progress of the patient’s individualized treatment plan.
  • Provides individual, group and family process-oriented counseling sessions.
  • Conducts educational, after care support and other didactic groups for patients and family members using various professional treatment modalities.
  • Participates in regular clinical team meetings/staffing and provides input on the psychosocial, disease management and case management needs of the patient.
  • Demonstrates an understanding, and adheres to all procedures and legislation, which apply to patients cared for in a Mental Health and Substance Abuse treatment facility.
  • Demonstrates a comprehensive knowledge of community resources and medical allied staff expertise to assure an appropriate discharge liaison is implemented.
  • Reports to community/state agencies any patient issues that are required by hospital policy and state or federal law.
  • Serves as the facilities representative in legal proceedings resulting from said reports.
  • Maintains accurate and timely medical records including psycho-social assessments, family session and daily progress notes.
  • Schedules, attends and documents court hearings as required, such as involuntary admission, guardianship, etc.
