About Muir Wood Teen Treatment Muir Wood Teen Treatment is a leading provider of residential and outpatient behavioral healthcare for teens ages 12–17. With programs in Sonoma County, Clovis, and Riverside, we specialize in treating primary mental health and co-occurring substance use disorders. The Clinical Case Manager/Discharge Coordinator plays a central part in ensuring that each client leaves residential treatment with a clear, well-coordinated plan for continuing care, working closely with the clinical team, families and external partners. This is a part-time position (24 hours per week, 3 days onsite) at our Riverside residential program and is eligible for BBS client-centered advocacy hours for associate clinicians. We are open to a flexible schedule; a minimum of 3 days on site. Requirements Essential Functions and Responsibilities: Lead the development and completion of individualized continuing care and discharge plans prior to client discharge Collaborate closely with therapists and Clinical Directors to determine appropriate recommendations for ongoing care Partner with the Outreach team to identify and coordinate specific referral options aligned with treatment recommendations Work directly with clients and families to ensure continuing care plans, appointments, and next steps are clearly understood and in place prior to discharge Ensure accurate and timely clinical documentation and communication with referring health plans, including Kaiser, IEHP, and others, in accordance with requirements, details, and deadlines Maintain close, ongoing collaboration with therapists, Clinical Directors, and external partners to support continuity of care and meet clinical and payer expectations This role is ideal for someone who values follow-through, clarity, strong systems, and collaboration as essential components of quality care. Qualifications: One of the following: An ASW or other associate-level licensed clinician eligible for supervision hours OR a non-licensed professional with robust experience in discharge planning, care coordination, or utilization management within residential, inpatient, or acute care setting Additionally, you: Are comfortable working in close collaboration with licensed clinicians to support clinically led treatment and discharge decisions Have experience or strong interest in adolescent mental health and/or substance use treatment Are organized, detail-oriented, and able to manage multiple moving parts and deadlines Communicate clearly and professionally with families, providers, and health plans Bring steadiness, professionalism, and follow-through to complex or time-sensitive situations Salary Description $32.00-$38.00 per hour

Requirements

  • One of the following: An ASW or other associate-level licensed clinician eligible for supervision hours OR a non-licensed professional with robust experience in discharge planning, care coordination, or utilization management within residential, inpatient, or acute care setting
  • Are comfortable working in close collaboration with licensed clinicians to support clinically led treatment and discharge decisions
  • Have experience or strong interest in adolescent mental health and/or substance use treatment
  • Are organized, detail-oriented, and able to manage multiple moving parts and deadlines
  • Communicate clearly and professionally with families, providers, and health plans
  • Bring steadiness, professionalism, and follow-through to complex or time-sensitive situations
