Under the direction of the Manager of Case Management and in collaboration with the hospital leadership team, has responsibility for daily operational management of the Care Management teams, including orientation and ongoing staff development. Is responsible for the planning, directing and team oversight of Care Management implementation at The University of Kansas Hospital. The Supervisor provides direction to the Integrated Nurse Case Managers and Social Work Case Managers. The primary role of the case managers is to collaborate, communicate and facilitate coordination of services during hospitalization and post-hospitalization as established by the healthcare team. The primary role of the social workers is the for the psychosocial assessments of patients, for post hospital discharge needs including home care, nursing home placement, durable medical equipment, financial assistance, counseling, and other community resources. Responsible for supervising approximately 15 Nurse Case Managers and/or Social Workers. Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.
