Requirements

  • Licensed Vocational Nurse or Licensed Practical Nurse with current licensure to practice nursing in the state.
  • Complies with accepted professional standards and practice. Possesses and maintains current CPR certification.
  • Demonstrates excellent observation, verbal, and written communication skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the agency requirements.

Nice To Haves

  • Two years’ experience in healthcare or home care setting, preferred.

Responsibilities

  • Participates in care coordination with agency staff and contractors.
  • Completes workflow tasks, administrative tasks, and other assignments to the Clinical Care Manager-LN responsible position in the agency’s electronic medical record.
  • Receives and enters orders from all relevant physicians, nurse practitioners, and/or physician assistants involved in the plan of care and ensures the orders are approved by the responsible provider.
  • Assists in the entry of patient referrals as needed.
  • Follows up on pending referrals to ensure timely patient admission in collaboration with the intake coordinator.
  • Prepares for and participates in case conference in collaboration with other healthcare team members and participates in decision-making regarding patient recertification/discharge.
  • Reviews patient occurrence reports and takes appropriate action.
  • Reviews patient infection control reports to ensure appropriate action has been taken and items are reported to the QAPI committee.
  • Performs in-home visits as assigned and completes electronic visit documentation in accordance with agency expectations.
  • Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
  • Prepares clinical notes in accordance with professional licensing body and internal policy using the company’s electronic medical record and submits documentation timely.
  • Ensures effective communication with all physicians and other healthcare providers involved in a patient’s plan of care.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Monitors systems and identifies problem areas to branch director both verbally and through written reports.
  • Meets mandatory continuing education requirements of the agency and licensing board.
  • Participates in agency-sponsored in-service training.
  • Performs other duties and activities as delegated by the branch director.
