Requirements

  • Current RN license in state of practice.
  • Graduate of an accredited nursing program (BSN preferred).
  • Minimum 2 years of hospice or home health experience.
  • Current CPR certification.
  • Valid driver’s license and reliable, insured transportation.

Nice To Haves

  • Prior leadership or supervisory experience preferred.

Responsibilities

  • Oversee and coordinate patient care provided by agency staff and contracted providers to ensure quality, safety, and continuity of services.
  • Manage physician orders, documentation, referrals, and workflow tasks within the electronic medical record.
  • Collaborate with Intake to support timely admissions and follow up on pending referrals.
  • Lead development and updates of individualized plans of care in partnership with patients, families, and the interdisciplinary team.
  • Monitor patient status, review reports (occurrence, infection control, OnCall), and take appropriate action.
  • Participate in IDG meetings and support clinical decision-making.
  • Perform in-home visits as assigned.
  • Ensure compliance with state and federal regulations and agency standards.
  • Communicate effectively with physicians, staff, and leadership, escalating trends or concerns as needed.
