Requirements

Current RN license in state of practice.
Graduate of an accredited nursing program (BSN preferred).
Minimum 2 years of hospice or home health experience.
Current CPR certification.
Valid driver’s license and reliable, insured transportation.

Nice To Haves

Prior leadership or supervisory experience preferred.

Responsibilities

Oversee and coordinate patient care provided by agency staff and contracted providers to ensure quality, safety, and continuity of services.
Manage physician orders, documentation, referrals, and workflow tasks within the electronic medical record.
Collaborate with Intake to support timely admissions and follow up on pending referrals.
Lead development and updates of individualized plans of care in partnership with patients, families, and the interdisciplinary team.
Monitor patient status, review reports (occurrence, infection control, OnCall), and take appropriate action.
Participate in IDG meetings and support clinical decision-making.
Perform in-home visits as assigned.
Ensure compliance with state and federal regulations and agency standards.
Communicate effectively with physicians, staff, and leadership, escalating trends or concerns as needed.
