About The Position

The Clinical Care Leader (CCL) will work as an integral member of the inter-professional collaborative practice team administering community-based strategies aimed at improving the health of the community through education, health screening and coordinating evidence-based programming.

Requirements

  • Bachelor's degree in Nursing strongly preferred in Fargo, Fargo Network, Bemidji, Sioux Falls Network and Rapid City. Required in Bismarck and Sioux Falls (Sanford USD Medical Center and Clinics).
  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies, licensure and certifications.
  • Basic Life Support (BLS) for health care providers required or within the first 60 days of hire required.

Responsibilities

  • Responsible for utilizing the nursing process to provide individualized nursing care to designated area which may include non-structured community or home-based settings.
  • Develops and maintains relationships and support from the local and regional community; including churches and other service organizations to promote capacity building, referring to the identification, strengthening and linking of your community's tangible resources, such as schools, businesses and local service groups, and community spirit.
  • Recruits, trains and retains community volunteers.
  • Understands the interconnectedness of spirituality/faith and health, holistic nursing assessment and care of the whole person; body, mind and spirit.
  • Utilizes technology to enhance quality patient care, collaborate with the inter-professional team and serve in leadership roles on tasks forces and committees.
  • Demonstrates critical thinking skills to analyze data and appropriately respond to changing or difficult situations.
  • Maintains role specific competencies; point of care testing, as well as competencies required for Registered Nurse (RN) role.
  • Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
  • The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.
