About The Position

The Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the hospice setting. This position is based in our Hastings Office. The CCL collaborates with other inter-professional colleagues to plan, implement and evaluate care, documents patient history, symptoms, medications and care provided in compliance with applicable standards/regulations, and utilizes technology to enhance quality patient care. The CCL effectively delegates and supervises unlicensed personnel, assumes overall nursing accountability for a patient assignment, and serves in leadership roles on department/organizational task forces and committees. The role requires strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations, coordination of care, patient assessment, patient education, triage, and various other nursing interventions.

Requirements

  • Bachelor's degree in Nursing strongly preferred.
  • Graduate from an accredited nursing program.
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing.
  • Obtains and maintains required department specific competencies and certifications.
  • Basic Life Support (BLS) required.

Nice To Haves

  • Certificate for Certified Hospice and Palliative Nurse (CHPN) preferred.
