About The Position

The Clinical Care Coordinator at Columbus Regional Hospital plays a vital role in integrating and coordinating patient care from entry into the healthcare system through discharge. This position collaborates with a multidisciplinary team, including a Patient Resource Specialist and a Social Work Specialist, to ensure optimal patient outcomes through effective communication and care facilitation.

Requirements

• Associate of Science in Nursing; Bachelor of Science in Nursing required within 3 years of hire or at least 10 years of direct experience.
• Indiana License as a Registered Nurse.
• Basic Life Support (BLS) Certification required within 30 days of hire.

Nice To Haves

• Three or more years of strong clinical experience in a clinical practice area.
• Knowledge of utilization review and InterQual.

Responsibilities

• Integrate and coordinate utilization management, care facilitation, and discharge planning functions.
• Communicate with patients, family members, and medical staff through care conferences and rounds.
• Collaborate with social services and outside organizations, including insurance and community resources.
