The Clinical Care Coordinator serves as a key member of the patient care team across Boulder Community Health (BCH) clinics, supporting care coordination, quality initiatives, and value-based programs. This role focuses on connecting patients—especially those with chronic conditions or high healthcare utilization—with appropriate care and community resources, while promoting return to primary care. The coordinator also facilitates smooth care transitions after emergency department visits and collaborates with interdisciplinary teams to enhance patient outcomes and reduce avoidable high-cost care. BCH primary care physician clinics are guided by the philosophy of the Patient-Centered Medical Home. The medical home organizes care around patients, while engaging a multidisciplinary care team working proactively to coordinate and track care for each individual patient. This position is full time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am – 4:30 pm MST. It may be necessary, given the business need, to work occasional overtime. We offer on the job training. The hours during training will be aligned with your schedule.
