The Clinical Care Coordinator works in a multi-disciplinary team to provide strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, wellness groups & activities, crisis intervention, end-of-life-planning, and family support. Services are performed in a variety of non traditional settings at various locations in the community, and in the office. SHIFT: This position is Tuesday to Saturday 8-4:30pm BACS clinical care coordinators get 40 hours per week of service hours toward licensure, 1 hour of individual supervision and 2 hours of group supervision per week.
