About The Position

Treasure Coast Community Health (TCCH), a long-established and highly respected healthcare provider with locations throughout Indian River County is continuing to expand. We are now seeking a Clinical Care Coordinator to be based at one of our growing patient care sites, to provide clinical, casework and group work services to patients and their families. Will perform a variety of duties to include assessing cases and preparing and implementing treatment plans, counseling and advocacy for patients, coordinating the provision of social services and working collaboratively with medical staff. As a non-profit Federally Qualified Health Center, our focus is meeting the medical, dental and mental health needs of the insured, non-insured and under-insured people in our community and to provide accessible, cost-effective, high quality, comprehensive healthcare throughout Indian River County, Florida .

Requirements

• RN or LPN certification required
• Excellent computer skills
